• Required COVID-19 Liability Waiver & In-Studio Attendance Agreement Form

  • This form MUST be thoroughly read, completed, digitally signed, and submitted before any student can physically enter the studio for a class, appointment, or lesson. It only has to be filled out ONCE, but understood that the guidelines this form details must be honored EVERY TIME the individual will be on-site at the Sun Salutations Yoga & Wellness Center location. If there are any changes, such as getting the vaccination after completing this form, I will resubmit another form with the new information.

  • I acknowledge:

    The contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. 

    Nichole K. Tracy, B.S.Ed., RYT-500, CPT and Sun Salutations Yoga & Wellness Center has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19 to keep attendees as safe as possible.

    I further acknowledge that Nichole K. Tracy, B.S.Ed., RYT-500, CPT and Sun Salutations Yoga & Wellness Center cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, studio staff, and other studio clients and their families.

    I voluntarily seek services provided by Nichole K. Tracy, B.S.Ed., RYT-500, CPT and Sun Salutations Yoga & Wellness Center and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment/class/lesson.

    I attest that:

    *I am not experiencing ANY symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

    * I have not traveled to a highly impacted area within the United States of America or anywhere internationally within the last 14 days.

    * I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

    * I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities.

    * I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19 by participating in social distancing and wearing a mask in public places.

    I agree to attend classes/lessons/appointments at the studio following the required precationary measures:

    *Pre-registration for classes is required (online form, text, or call), as on-site yoga class numbers are now limited to FOUR participants, and this form MUST be read, completed, and submitted before I enter the studio (ONLY ONCE, but honored EVERY TIME). There are NO DROP-INS for in-studio classes at this time. If you cannot attend class in-person, all yoga classes will be shared LIVE via Zoom on the 'Yoga Instruction' page of the studio website.

    *I will WEAR A MASK when inside the studio, including before, during, and after yoga practice and exercise (when social distancing requirements cannot be met), and during Thai Yoga Bodywork appointments.

    *The studio will NOT be providing any yoga mats or yoga props during class, therefore, I will BRING MY OWN mat and yoga props, then take them with me when I leave.

    *I agree to have my temperature taken before class with a touchless forehead thermometer.

    *I will keep a 6' minimum distance at ALL TIMES from all other participants.

    *I will cover my mouth with my arm when I cough or sneeze, and will be diligent about my hygene, making sure my hands are thoroughly washed/sanitized, and avoid touching my face. 

    *I will bring minimal personal belongings into the studio, and take them all with me when I leave.

    *I will sanitize my hands and anything I touch in the studio with the provided cleaning/sanitizing solutions.

    *I will not share any equipment with other students/participants.

    *I understand that hands-on and close contact adjustments will NOT be given by the teacher/trainer during yoga or personal training.

    *I must bring my own water/beverage, as they will not be provided by the studio.

    I hereby release and agree to hold Nichole K. Tracy, B.S.Ed., RYT-500, CPT and Sun Salutations Yoga & Wellness Center harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from Nichole K. Tracy, B.S.Ed., RYT-500, CPT and Sun Salutations Yoga & Wellness Center. I understand that this release discharges Nichole K. Tracy, B.S.Ed., RYT-500, CPT and Sun Salutations Yoga & Wellness Center from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Nichole K. Tracy, B.S.Ed., RYT-500, CPT and Sun Salutations Yoga & Wellness Center. This liability waiver and release extends to the studio together with all owners, partners, and employees.
