Liability/Medical Release/Release of All Claims

I am the parent/legal guardian of the youth listed above and agree to:

hold harmless PINE VALLEY UNITED METHODIST CHURCH and the Director(s), staff, and members thereof from any and all liability, claims, or demands for personal injury, as well as damage and expenses, of any nature that may be incurred by the parent/guardian and my child that may occur while he/she/they are participating in PINE VALLEY UNITED METHODIST CHURCH Youth Ministry Activities.

I, on behalf of my child, assume all risk of personal injury, damage, and expense as a result of participation in recreational activities involved.

Authorization and permission are given to said church to furnish any necessary transportation, food, and lodging for our child/participant.

I, as parent or legal guardian of the child(ren), give permission for full participation in the trip/activity including but not limited to water activities and off-site trips.

I give permission to take my child to a doctor or hospital and authorize medical treatment, including, but not limited to, emergency surgery or medical treatment, and assume all responsibility for all medical bills if any.

Should it be necessary for my child to return home due to medical reasons, disciplinary action, or otherwise, I assume all transportation costs.

Media Release Authorization

I hereby grant permission to PINE VALLEY UNITED METHODIST CHURCH and its subordinates to post a photograph and/or video of my child on the PINE VALLEY UNITED METHODIST CHURCH website and/or social media pages and a photograph of my child in PINE VALLEY UNITED METHODIST CHURCH printed material. I understand photos will not be labeled with names.

(if participant is under 18): I represent that I am the parent/guardian of above listed youth who is/are under the age of 18. I have read the above media release authorization and am fully familiar with the contents thereof. By signing this form, I agree to the above Media Authorization.

A Word From Pastor Chris

   I want to say we are excited for the new year to start!!!  We have a tremendous team of staff, counselors, volunteers, and teachers ready to minister to the youth of our congregation as well as the community.  I want to offer my sincere gratitude for you committing to get the youth to all events on time as scheduled.  I know things will be a little different to start our year but I see a revival coming to this youth group and I am excited for your entire famlies to be involved.  There will be a lot of schedule updates coming on a regular basis this year so stay tuned to pvumc.net and our church Facebook page.  You will also receive our Remind invite very soon. Parents please contact me anytime with questions at Chris@pvumc.net or my cell 910-232-0068.  I look forward to connecting the hearts of our youth to Christ and to one another.  God bless you all.
