All adopting parties are required to complete this form prior to adoption. AHHA reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!

  • Cat adoption fee may vary depending on age and breed, and includes age-appropriate vaccines, spay/neuter, deworming, flea medicine, microchip.*
  • ***IMPORTANT*** I understand there may be multiple applications for a kitten/cat and submitting an application does not guarantee I will be selected.*
  • In lieu of a home visit during these uncertain times, we may ask for a video visit via FaceTime or Zoom so we can ensure a safe environment for your newly adopted pet.*

    By submitting this application I understand that I assume full responsibility for the welfare of this pet from the date of adoption and that all above information is true and accurate to the best of my knowledge.

    By Submitting this application you hereby agree that you will not use this animal in any sort of fighting, breeding, or cruelty. You will love and care for this animal for the rest of it's life, and you are ready to commit to that. You also agree that if for any circumstances you can longer provide a loving and safe home you will contact an admin of the rescue before taking the animal to a shelter or re-homing on your own.
