Completion of this form is mandatory for ILAC and ILAC International College, and Georgian@ILAC students.

*Students attending Canadian Tourism College or Fanshawe Toronto, If you're requesting ILAC Accommodation Service, It is mandatory to complete this form.
- This form can be completed by the student's parent, legal guardian, or agent.
- If you are an agent completing this form on behalf of a student, please note an ILAC representative will contact you by email.

To complete this form, you need to have the following documents:
1. A unique Student Number can find in the LOA or Invoice
2. Flight information (upload require) Student Name, Arrival and Departure flight information will be reviewed.
3. A picture of the student (Size:640pixels X 640pixels is maximum) for Student Attendance
