Howard College Sexual Misconduct Online Reporting Form
This form is intended to be used as an option for a student or an employee of Howard College to electronically and anonymously report an allegation of sexual misconduct committed against or witnessed by the student or employee. This method of reporting is not for emergencies or crimes in progress. Always dial 911 for emergencies. When submitting this form it is important to understand you are not filing a police report. Sexual misconduct / assault can entail physical, verbal, written, audible, visual and/or electronic activity and includes but is not limited to: sexual violence; sexual harassment; sexual exploitation; dating/intimate partner violence; domestic violence; stalking; sexual coercion; and sexual exploitation as defined by state and federal law and Howard College policy. Please provide as many details as possible to assist with the investigation process. When submitted, this form will be sent to the Title IX Coordinator.
