TPN adoption application

Tiny Paws Nicu is a private independent rescue ran by me, a 19 year old girl! I am not a 501c3 rescue.

Please read to the bottom before filling out this application, this information is important!

You must have a veterinarian to list! If you do not have a current veterinarian, you must find one and get established! Cats require annual vet exams, vaccinations etc.

Note that,

completion of this application does not guarantee adoption of the kitten/cat you've applied for. Answers provided will determine if you're the purrfect fit for the cat/kitten. Please do not get upset if your application is denied, it's most likely the simple fact that your household and lifestyle isn't a good fit for the kitten/cat, or that they do not match the description of what you're looking for in a kitten/cat.

I require a $30 non refundable deposit of the adoption fee once your application is approved, if you back out on the adoption your deposit will be considered a donation towards the kittens care. Deposits will be sent through Venmo or Cash App. The rest of the fee ($45) will be paid on adoption day. If you decide to send the full amount at the time of deposit it cannot be refunded as it will most likely be put to immediate use for vet care towards the kitten/cat.

by filling out this application I have permission to contact your vet for references along with your landlord if you have one.

completion of this application gives me permission to take any kitten/cat back from a placed home if i feel it is not a good match, their needs are not being met, and or if I feel they're not being properly taken care of.

If it does not work out, and or isn't a good fit for your household/family, you must return them! I will understand, and no questions will be asked.

It is required that you return them if you can no longer keep them, no matter the time frame. Adoption donations are non refundable.

Do NOT go behind my back and rehome or surrender the cat/kitten. If you do not follow rules and return the kitten/cat I will get legal action involved to get the cat/kitten back in my care.

I do not adopt out un vetted / un fixed cats! The kittens will be 3+ months old when they are ready.

Please make sure everyone in your household is on the same page with wanting to adopt a cat before filling out this application.

Adoption Fee:

$75 for one kitten $125 for a pair.

includes the following:

• Spay / Neuter

• Dewormer (2+ rounds)

• Flea & Earmite Treated

• FVRCP (2-5 vaccines)

• Rabies (1 year)

• FIV/FeLV tested

• Microchipped

Adopters will get a file with all of the kittens information, with proof of everything. Spay/Neuter certificate, vaccine/dewormer cards, microchip sticker etc. We keep copies on hand if we need to fight to get the kitten back in our care.

Microchip information is registered under TPN but we will add you as the owner on our account.

You will be required to sign contracts on adoption day. When your application is approved you will be sent the contracts via text or email so you can read over it and then sign on adoption day.

We love picture updates! Don’t be afraid to send us as many updates and pictures as you take! It makes our day! We raise these kittens from horrible shape, we spend a lot of time with them making sure they make the perfect companions and are healthy. Saying goodbye is hard, but seeing that they are happy, heathy, and safe makes this all worth it!
