  • GUIDELINES FOR REGISTERING FOR SWIM LESSONS

    Please read the following guidelines for tips on filling out your registration form

    Classes Option:

    Consists of 10 lessons with a maximum 12 participants per class (generally 6 – 8 participants)

    ·        choose which level you are enrolling each participant in

    OR

    Private Lesson Options:

    ·        Choose one of the following per participant:

     1.      Private - $49.50 per hour – one participant in the pool

    a.      choose how many hours each participant is enrolling in, based on the recommended hours in the chart below

    b.      indicate which Program level each participant is enrolling in

    2.      Semi-private - $77.50 per hour – up to 4 participants (cost can be split between participants)

    a.      choose how many hours your participant is enrolling in, based on the recommended hours in the chart below

    b.      indicate which Program Level your participant is enrolling in

    c.      List the other participants who will be sharing the same session and how the cost will be split if more than one family paying

    3.      Adult - $115.00 per hour

    IMPORTANT NOTE: We are transitioning from Red Cross curriculum to Swim for Life curriculum. The Transition Chart is included to assist in choosing the correct level to enroll this season.

     PRESCHOOL POLICY

    The Alsask Swimming Pool will enroll children the year they turn two, on the following conditions:

    ·   Child must be potty‐trained or wear a swim diaper.

    ·   There must be absolutely no interference from the parents or guardians.

    ·   If the child cries, the parent or guardian will be required to leave the pool deck area, until such time as the child stops crying and the instructor allows the parent to return.

    ·   The Preschool program implements a controlled dunk of the child and parents must allow for the child to participate in the controlled dunk.

     

    I have CAREFULLY READ the Preschool Policy and I, the undersigned, hereby agree to allow my child to participate in the Preschool program.

     PHOTO RELEASE WAIVER

     

    Photos and videos of participants and patrons of the Alsask Swimming Pool are used to promote andshare information about our amenities and programming. The RM of Milton No. 292 retains photos inour archives and photos may be altered, copied, published or distributed to local newspaper or mediafor use in their publications and promotional materials. By providing your consent, you grant the RM ofMilton No. 292 Council to use you and/or your child’s(ren’s) likeness in photographs, videos, or otherdigital media in any of our publications, including online, without compensation.

  • AUTHORIZATION FOR PROGRAM PARTICIPATION, RELEASE AND WAIVER OF CLAIM,

  • 1. I, the undersigned, hereby agree to allow the individual(s) named to participate in the Alsask Swimming Pool programs listed above.

    2. I certify that, to the best of my knowledge, the participant(s) named herein is/are physically able to engage in these activities.

    3. In registering myself or my child/ward to participate in the program(s), I am waiving and releasing all claims for injuries that I or my child/ward may sustain arising from the attendance of the program(s), including personal injuries or property loss. I agree that by signing this Risk Acknowledgement, Release, Waiver of Claim and Assumption of Risk, as a parent or guardian of a participant who is under the age of 18, I acknowledge and assume that there are certain risks and hazards inherent in the program to which I am willing to expose my child/ward. I will pay for any and all costs incurred by the Alsask Swimming Pool, the Rural Municipality of Milton No. 292, its employees or agents should a claim be made or an action commenced on me, my child’s/ ward’s behalf. I certify that I assume and will pay my own or my child/ward’s medical/emergency expenses. If the Alsask Swimming Pool staff, at its sole discretion, deem it necessary to call 9‐1‐1 for my and/or my child/ward’s health and safety in the event of an accident, illness, or other incapacity; regardless of whether I have authorized such expense and that I shall be financially responsible for the cost of any such advice and services.

    LESSON POLICY

    Once registered and paid, there will be no refunds issued. If withdrawal from a session becomes necessary the registration fee may be transferred to a different session with the same or lesser monetary value and must be held at the pool during the current season, subject to available openings.

    I have CAREFULLY READ the Authorization for Program Participation, Release of Waiver of Claim, and Assumption of Risk, and I fully understand and I am freely signing to indicate my agreement.
