Consultation Request Form

Thank you for showing interest in getting tattooed by me! This is a form for making the booking process easier and faster for both, you and I. 

GENERAL INFORMATION

PLEASE READ *EVERYTHING*

SO YOU DON'T WASTE BOTH OUR TIME

• Filling out this form does not guarantee you will be chosen and contacted to come in for a consultation and tattoo.
• If chosen, you will be contacted via text message. The response may be immediate, but could also take weeks or months depending on current circumstances and availability. Be patient.
• Being chosen for a consultation does not guarantee you will be booked for a tattoo. That final decision will be decided at the consultation.
• I am sometimes booked months in advance. Please keep this in mind when you come in for your consultation. Your wait time for your actual tattoo may be months.
• If you are given an appointment time for a consultation and you no-call-no-show, you will be declined any future service.
• I currently work at Assassin Tattoo Studio located at 1900 W. Market St, Akron.
• The front desk is open from 12p-8p every day except holidays.
• I am rarely available Saturdays or Sundays, but may have limited availability on those days. However, I charge more hourly for weekends and evenings.
• I currently charge $150/hr on weekdays from 9a-4p, and $200/hr on weekends and evenings.
• I typically schedule my appointments around 9am, before the shop opens, and I rarely work late weekends, afternoons or evenings
• To get a price estimate, bring any and all references you have to your consultation.
• Unfortunately, I do not estimate prices for anything through text or messages due to too many variables. All price estimates are given in person.
• For the time being, I’m not doing any:
  • color
  • walk-in tattoos
  • tribal/celtic 
  • album art
  • mandalas
  • any overdone imagery
  • copies of other artists' work
• Tattoos I love doing:
  • portraits
  • skulls human and animal
  • hands
  • flowers
  • nerdy stuff
  • dark subject material
  • detailed pieces
  • photo realism
  • projects that give me artistic freedom
• If the tattoo you want is something I may not be comfortable with doing, I can refer you to one of our other great artists to ensure you get the best tattoo possible. 
• The required deposit for all appointments is $100 cash. 
• All deposits/drawing/appointment fees are NON-REFUNDABLE. Under no circumstances will you be refunded your deposit. NO EXCEPTIONS!
• The deposit WILL go toward the cost of the tattoo, unless specified otherwise. The deposit price will be taken off of the price at the end of the last session if there are multiple sessions.
• The deposit cannot be transferred to another artist or another client.
• The drawing will be shown to you the day of your appointment. No sooner. If any changes need to be made, you may be asked to reschedule.
• The artist owns all rights to the artwork, and they are not obligated to release a drawing.
• Any ideas post consultation/inquiry, regarding the design, must be made at least 2 days prior to the appointment.
• If major changes to the design are made, you may be asked to come in for a second consultation. This may affect the scheduling of your tattoos.
• You understand that your artist has a specific style and has limitations.
• You are expected to send any and all final reference materials and a brief description of your tattoo to your artist no later than 48 hours before your appointment. If it is received less than 48 hours before your appointment, you may be required to reschedule.
• If you become sick or pregnant before your appointment, you must reschedule. Deposit will be forfeited if you arrive for your appointment intoxicated, sick or without valid ID and cannot be tattooed.
• I cannot tattoo damaged skin. If your skin is scraped/scratched, sunburned, peeling, broken out, razor burnt, bruised or otherwise damaged in any way, you must reschedule or your deposit may be forfeited.
• Do NOT attempt to numb your skin before your appointment or you may be asked to leave and forfeit your deposit. Some numbing agents can cause the skin to become difficult to work with.
• I will not take unnecessary risks when it comes to the health and safety of my clients, you will be asked to reschedule if I don't feel confident that I can safely apply a tattoo to your skin.
• Deposit will be forfeited if more than 6 months pass without communication or attempt to schedule.
• Deposit will be forfeited if you do not complete your tattoo within 2 years.
• Deposit will be forfeited if you decide not to get tattooed.
• Deposit will be forfeited if you have to reschedule more than once
• If you have time of budgetary restrictions, you must let your artist know within 48 hours before your appointment or you may have your appointment rescheduled and may forfeit your deposit.
• All cancellations / reschedules without 24 hours notice to scheduled appointment date will forfeit their deposit.
• No-call / no-shows also forfeit their deposit and may be declined any future service.
• Do not shave yourself. I will shave you before I tattoo you.
• If you’re more than 15 minutes late for your appointment, you will forfeit your deposit and may be declined future service
• Do not expect to be tattooed immediately upon arrival. The set up and last minute design process may take time. Be patient and prepared.
• If you need to reschedule, you can do so at any time, but understand that your new appointment date could be months later.
• Your artist may need to reschedule your appointment for their own personal reasons. A new date for your appointment could be weeks/months later.
• Photos and video may be taken during or after the procedure, and by submitting this form, you agree to have any form of photography taken of you and uploaded to social media and other websites for the purposes of advertising, networking and promotion 
• Dress appropriately. Don’t wear anything you don’t want ink to get on. Wear something that makes the body part being tattooed easily accessible.
• Bring a drink with a lid and maybe a snack to eat during breaks.
• Bring headphones to listen to your own audio
• You can bring one person with you to your appointment. No more than one, and no one under the age of 18.
• No animals are allowed in the shop with the exception of service animals with proper documentation.
• Privacy is available and pasties are provided if you wish to cover yourself in the circumstances where you may have to expose private parts of your body.
• The shop is cash only. Bring enough with you to pay for your tattoo. The final cost of the tattoo my vary largely based on size, detail, difficulty level, skin consistency, client movement, etc.
