Agreement of Release and Waiver of Liability for All the People

I First NameLast Name hereby affirm that I am voluntarily starting a course of instruction by the instructors working at or with All the People in physical fitness, health and yoga. I am voluntarily participating in the activity entirely at my own risk.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the yoga or physical instruction. I represent and warrant that I am physically fit and have no medical conditions that would prevent my full participation in the event or class.

I understand if I am pregnant or have any other health condition, I will take necessary steps to ensure my doctors and health care providers know I am participating in this event. I assert that I am of fit health to participate in this class or event and will alert all teachers, instructors, volunteers or employees if I am pregnant or have a heath condition.

In consideration of being permitted to participate in this event, I agree to assume full responsibility for any risks, injuries or damages, know or unknown, which I might incur as a result of participation in the event or class.

In further consideration of being permitted to participate in the event, I knowingly, voluntarily and expressly waive any claim I may have against Jennifer Haas and All the People, for injury or damages that I may sustain as a result of participating in the class, event and instruction. I, my heirs and legal representatives forever release, waive, discharge and covenant not to sue Jennifer Haas or All the People for any injury or death caused by their negligence or other acts. I agree to indemnify and hold harmless Jennifer Haas or All the People for any action legal or otherwise created based on my participation in the yoga classes, event or instruction.

I authorize my photograph, picture and voice to appear in any promotion, advertisement, documentary, television, video to radio coverage of All the People without compensation. I also authorize the use of my name in connection with these recordings.

I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge the trainer, instructor, employee or volunteer from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage.
