Dear Teacher: We are requesting an evaluation of the above student. The above student has applied to the Upward Bound Math/Science program at Frostburg State University. UBMS is a pre-college program that encourages qualified students to further their educational pursuits beyond high school. www.frostburg.edu/ubms

Two of the most important factors in considering students for UBMS are post-secondary/college potential and a need for our program's services. A student may or may not be living up to that potential; however, that potential must be present. Your realistic evaluation will of great assistance to UBMS in determining the student's suitability for our program. When evaluating, keep in mind the following criteria: academic strengths and weaknesses, the potential for growth, behavior in class, level of motivation, work quality, and achievement. This evaluation does not necessarily mean you are recommending the student. We are looking for a realistic evaluation of the student's potential for success in post- secondary education in math and science fields of study.
