By signing this form, I give permission to my child's teacher and the Nevada Learning Academy staff to speak with the above named Learning Coach regarding my child's attendance, grades, and behavior, and to view and/or receive copies of my child's education records, for the 2023-2024 school year. I understand that if my child's Learning Coach changes, it is my responsibility to contact Nevada Learning Academy to update the information provided in this form. I further understand and agree that if my child's Learning Coach changes and I do not update this form, neither Nevada Learning Academy nor the Clark County School District will be responsible for the disclosure of documents and/or information to the Learning Coach identified in this form.
