Waiver of LiabilityWAIVER OF LIABILITY, RELEASE, ASSUMPTION OF RISK & INDEMNITY AGREEMENT We, the undersigned Player and Parent(s) or Legal Guardian(s), hereinafter collectively called “Participants,” for and in consideration of the Player’s registration with the Eastside Elite Hockey League, Inc. (“EEHL”) and of the Player’s being allowed to participate in EEH: events, agree as follows: WAIVER OF LIABILITY & RELEASE Participants waive any and all liability for any cause of action against EEHL, its officers, Board members, the managers and coaches, officials, sponsors, facilities, and all those in participation with any of them (collectively called “Releasees”), for personal injury, property damage, or wrongful death occurring to Players arising out of participation in the EEHL events, team activities, the sport of hockey, and/or activities incidental thereto, whenever or however any such injury or damage may occur, including, but not limited to, transportation to and from any such event, and for such period as said activities may continue; and by this agreement, any such claims, rights, and causes of action that the Player may have are hereby waived and relinquished. ASSUMPTION OF RISK Participants acknowledge, understand, and assume all risks inherent in the game of hockey and any EEHL events or team activities, and understand that said sport and related activities involve risks to The player’s person, including bodily injury, partial or total disability, paralysis, death, and damages which may arise therefrom, and the Participants have full knowledge of such risks. These risks and dangers maybe caused by the negligence of the Player or the negligence of others, including the Releasees. It is further acknowledged that there may be risks and dangers not known to Participants or are not reasonably foreseeable at this time. Participants agree to abide by and be bound under the rules of hockey and all rules and regulations and requirements of EEHL. Participants acknowledge, understand, and assume the risks, if any, arising from the conditions and use of hockey rinks, facilities, and related premises and acknowledge and understand that included within the scope of this waiver and release is any cause of action arising from the performance, or failure to perform maintenance, inspection, supervision, or control of said rinks and surrounding areas and for failure to warn of dangerous conditions existing at said hockey rinks, for negligent selection, negligent supervision or instruction by any of the Releasees. INDEMNITY AGREEMENT Participants agree that if any claim for a Player’s personal injury, property damage, or wrongful death is commenced against Releasees, Participants shall defend, indemnify and save harmless Releasees from any and all causes of action by whomever or wherever made or presented for Player’s personal injuries, property damages or wrongful death. It is the purpose of this agreement to waive and release Releasees from liability for personal injury, property damage, and wrongful death caused by negligence, including the negligence, if any, of the Releasees. Player and/or Player’s Parent(s)/Guardian(s) acknowledge that they have read the above paragraphs, understand Its terms and significance, and have not relied upon any representation of Releasees; and that they are fully Aware of the dangers of hockey. (Parent/Guardian must sign if Player is under age 18 at time of signing.)

Media Release Form Authorization for Release of Photographs, Video, and Audio Recordings In consideration of the participant’s registration and being allowed to participate in any manner in Eastside Elite Hockey LLC (hereinafter referred to as EEHL), programs, camps, training, activities, and related events, I hereby grant permission and consent that EEHL, its agents, officers, employees, sponsors, advertisers, and all other persons or entities to whom circulation or dissemination may be made including news and media organizations to use, reproduce, distribute, exhibit, broadcast, or otherwise disseminate, photos, video and audio recordings, or other forms of media existing now or in the future, of me and/or my child while appearing at the programs, camps, training, activities, and all related events, whether or not edited, retouched, altered, or otherwise changed or modified. I further consent and agree that such media shall become the sole property of EEHL, free and clear of any claims on my part. I hereby waive any right to inspect, edit or approve the media, whether its use is known to me or unknown. I further consent that this release includes the use of this media in any form and way deemed appropriate, which may include, instructional, educational, promotional, commercial, artistic, or entertainment purposes, without limitations or reservations. I further consent and authorize the use of this media, with or without my name or the name of my child, and without notice to me. I understand the media may be viewed by the general public and that other use may be made of the media. I further agree and consent that EEHL, its agents, officers, employees, and others are not responsible for any misappropriation of the media by any member of the general public or any other person or entity. I release EEHL, its agents, officers, employees, and others who get their rights through them, from any and all claims or actions I may make arising out of the use of this media including all personal and/or property rights, even if from their negligence, to the fullest extent permitted by law. I acknowledge that I may not terminate or rescind this perpetual release. I understand that my child’s participation is permitted in reliance upon and induced by my agreement to the terms herein. I acknowledge that I have not entered into this agreement in reliance upon any representation, written or oral, express or implied, not contained herein. This agreement shall inure to the benefit of and be binding upon the parties' respective heirs, successors, licensees, and assigns. This agreement shall be governed by and construed in accordance with the laws of the State of Michigan. I certify that I, as parent/guardian, have the legal capacity and authority to act for, or on behalf of the a minor child named herein. I do hereby execute this release on behalf of a minor child. I hereby bind myself, the minor child, and all respective heirs, successors, licensees, and assigns. I agree to indemnify and hold harmless the persons and entities referenced herein from any claims, actions and/or liabilities, which may arise against them as a result of, or arising out of my legal capacity or my authority to act for and on behalf of the minor child in the execution of this release and agreement or my execution thereof. I HEREBY AFFIRM AND ACKNOWLEDGE THAT I HAVE READ THIS DOCUMENT, FULLY UNDERSTAND THE CONTENTS THEREOF AND THE EFFECTS UPON MY LEGAL RIGHTS, AND AGREE TO BE BOUND THEREBY.
