  • I understand that completing this application does not guarantee I will be allowed to adopt a dog. It is a tool to help us match you to the right rescue and give them the best chance to thrive. I acknowledge that all information provided on this form is true and correct. I also certify that I have never been convicted of animal cruelty or neglect. 

    I agree to allow a representative from Paws and Hooves Animal Rescue or a partner rescue to contact my personal and vet references and agree to allow a home visit prior to adopting. 

    If I am approved to adopt, I agree to pay the adoption fee. I understand that all rescues adopted from Paws and Hooves Animal Rescue will be spayed/neutered (when age appropriate), heartworm tested, and/or treated, and be up to date on vaccines prior to adoption.
