Please take a good quality picture of the body part you would like to get the tattoo on. This is VERY IMPORTANT so I can see if there are any scars / other tattoos / freckles / moles / etc involved, which could lead to difficulties when you show up for your tattoo. If there is a cover-up involved, please take a well-lit photo of the tattoo or scar that you would like covered as well as some area around it. Also please note how old the tattoo/scar is in the description box.

I understand that this booking request form does not guarantee me an appointment. I understand that I will be contacted for follow up via email within 48 hours, and will be required to send a deposit via etransfer within 24 hours of agreeing to an appointment time with the artist. I acknowledge that until the deposit has been received my appointment has not been confirmed. I understand that this is a non refundable deposit. If I need to cancel or reschedule the appointment I will do this no later than 48 hours before my appointment to avoid losing my deposit. I understand that if I make changes to my design after submitting this request and booking the appointment the cost may vary from the quote I was given. *
