Portland Porpoise Membership Withdrawal Form
The following programs can use this form to withdraw and end future payments (note refunds will not be issued for payments made):

- Stations 1-7 Lessons

- Adult monthly members

- Following competitive groups enrolled in monthly: Porpoise Aspire, Ready, Challenge or MS Prep

The following programs should use this form to notify team of withdrawal Payments will continue per financial term agreements:

- HS Prep swimmers (financially commited to Sept-Nov enrollment)

- Porpoise Achieve & Performance swimmers (financially committed to Sept- July)

Submission deadlines:

For MONTHLY payment plans, form must be submitted by the 20th of the month to avoid the subsequent month payments.
