Off Season Golf Training Session 1

Full Session 8 Week Cost is $950 per session. Session 1 includes weekly golf and training classes. Select one Golf Class and one Golf Training class that will remain the same for the 8 week session. Session 2 will start in January. Golfers will also receive (1) 30 Minute Private Golf Lesson during the session from one of the golf professionals. Once you have selected one class for each section you can complete the registration with Credit Card payment below.

As the parent or legal guardian of the child named above, I hereby give my full consent and approval for my child to participate in the golf and training program listed above. I understand that there are certain risks of injury inherent in the practice and play of this sport, and I am willing to assume these risks on behalf of my child. I hereby certify that my child is fully capable of participating in the designated sport and that my child is healthy and has no physical limitations unless advised prior to the start of the program. In addition to giving my full consent for my child’s participation, I do hereby waive, release and hold harmless the organization named below, its officers, coaches, sponsors, supervisors and representatives for any injury that may be suffered by my child in the normal course of participation in the designated sport and the activities incidental thereto, whether the result of negligence or any other cause. I also grant Elevation Golf and Correct 2 Compete permission to take photographs and/or video of my child while participating in club activities to be used for training or social media advertising. The photographs and videos will be used for promotion through any or all of the following formats: Website, flyers, brochures, newsletters, presentations, Social Media and other marketing activities.
