The permissions agreed to in this form apply to the customer and all students listed herein and enrolled with DancEdge Emerald for the calendar year 01/01/2025 to 31/12/2025 inclusive. This medical authority includes permissions for all students listed in DancEdge Emerald family membership.
I hereby declare the following information is true and give permissions for all students listed in this DancEdge Membership: - The above listed dancers are in good health and are able to participate in classes run by DancEdge Emerald. - I give authorisation for a member of DancEdge Emerald to provide appropriate medical attention for my child in an emergency, including but not limited to: application of a Band-aid / Ice-pack / removal of splinter / cleansing of wound using saline solution. - I give permission for DancEdge Emerald to contact an ambulance and arrange for emergency medical treatment for my dancer if required, in the understanding that the teacher will make a concerted effort to contact the parental/guardian emergency details provided on enrolment.*

DANCE ACCOUNT AGREEMENT 2025

I hereby accept responsibility to pay owed fees to DancEdge Emerald according to their trading terms and conditions. I am aware of and agree to the following: - The Family Membership fee and Student Registration fees must be paid on enrolment and are NOT REFUNDABLE. - All fees are due and payable each Term within 14 days of invoice date, unless a payment arrangement is in place. In Term 4, fees must be paid in full by Photo Day for dancers to receive their costumes and participate in the Concert. - Notice of a student’s withdrawal from class MUST be given in writing to the Dance office, by no later than one week into the term otherwise fees for that term will be payable. - Fees are only refundable for medical reasons, and on written request to the Committee for approval. The Committee's decision is final. - If I do not pay my dance fees as per this agreement, DancEdge Emerald may transfer the debt to a debt collection agency and I WILL BE LIABLE for ALL fees and charges incurred by the debt collection agency in collecting my debt. - A General information sheet outlining all DancEdge conditions and expectations can be requested from the DancEdge office.*

RELEASE OF LIABILITY & ASSUMPTION OF FULL RESPONSIBLITIES FOR ALL RISKS OF BODILY INJURY, DEATH OR DAMAGES
I give my consent for myself/my students (as listed above and included in this membership) to participate in the curriculum / instruction taught by DancEdge Emerald. - I understand that it is the express intent of DancEdge Emerald to provide for the safety and protection of my dancer/s. - I understand that participation in the curriculum / instruction as taught by DancEdge Emerald may result in injury. Injuries may include, but are not limited to: muscle strains, muscle tears, broken bones and severe injuries such as paralysis and even death. - I agree to guarantee payment of any medical expenses incurred as a result of curriculum / instruction, rehearsal, performing or general participation in activities of DancEdge Emerald. - With regard to the use of facilities of DancEdge Emerald, I waive all rights or causes of action against the building owners, and/ or DancEdge Emerald for injuries or other damages suffered by my dancer/s and/or myself whist under the supervision of DancEdge Emerald. - I agree to release DancEdge Emerald and its employees from liability in the future. I hereby certify that this Acknowledgement of Risk and Waiver of Liability has been read by me, understood completely, and signed voluntarily.*
