Medical Consent I, the above pupil, give my written consent to receiving medication as instructed and any emergency medical, dental or surgical treatment, including anaesthetic or blood transfusion, as considered necessary by the medical authorities present. I understand the extent and limitations of the insurance cover which can be provided on request. I will inform East Dunbartonshire’s Senior Phase Partnership Programme if there is a change in any medical circumstances entered on this form or if I have been in contact with any contagious or infectious diseases in the 4 week period prior to each programme term start date.

College Consent I, the above pupil, give my written consent and permission to attend East Dunbartonshire’s Senior Phase Partnership Education Programme. I agree that I will travel to college by means of transport provided by East Dunbartonshire Council if available for my chosen course. If no such transport is provided, I understand I shall be responsible for making my own way to and from college. I understand that the cost of travel can be reclaimed by proof of a valid travel ticket using a Young Scot Card to claim a travel discount. Any tickets purchased not using the travel discount will be reimbursed at the discounted rate only. Travel tickets should be submitted to the school office and repayment of travel expenses to pupils will be made on a regular basis. I also confirm I will inform the school if I no longer wish to participate in the programme.
