• Paws and Hands United Adoption Application

  • In order to be considered to adopt a pet, you must:

    « Be 18 years of age or older;

    « If you are below 18 years, a parent or legal guardian should fill in the form;

    « Have the knowledge and consent of your landlord if you currently rent;

    « Have identification (of the same person who is filling out the form) showing your present address for verification purposes only;

    « Be able and willing to provide a loving, safe and peaceful environment for the pet;

    « Be able and willing to spend the time and money necessary to provide training, medical treatment and proper care for the pet.

  • CAT ADOPTERS ONLY

    You agree never to declaw this cat. You agree
    that declawing consists of amputating not just the claws, but the whole phalanx
    (up to the joint), including bones, ligaments, and tendons! Declawing is not a
    “simple”, single surgery but 10 separate, painful amputations of the third phalanx
    up to the last joint of each toe. You agree declawing can cause serious physical,
    psychological and behavioral complications. You understand that scratching is
    normal cat behavior and you will provide scratching pads and keep their nails.

  • Agreements of Approved Adoption

    I agree that the animal(s) is (are) being adopted for myself and will not be sold, adopted, or given to another party.

    I agree that the animal(s) that is (are) adopted will not be used for commercial or breeding purposes and I agree to sterilize the animal(s) if it (they) is (are) not sterilized at point of adoption. Adopter will also sign a spay/neuter contract agreeing to the sterilization of the pet should it not be old ebnough for sterilization at the time of adoption. If sterilization is not completed Paws and Hands United Inc. reserves the right to confiscate the pet from the adopter.

    I agree that the animal will not be allowed outdoors without proper supervision.

    I agree to care for the animal in a humane manner and be a responsible animal guardian. This includes supplying adequate food, water, shelter, attention, and medical care.

    I agree that if at any point I can not keep the animal, I will return him/her to Paws and Hands United Inc. without requesting a fee.

    I understand and agree that the current fosterer makes no guarantees about the animal's temperament and is not responsible for future damages or injuries caused by the animal.

    I give the current rescuer/fosterer permission to call my home at any reasonable time to assure that the animal is being properly treated and cared for.

    I agree that all statements I have made on this form are true. If it is found that any statements I have made on this form are not true the adopted animal can be confiscated.

  • I understand that animals may be unpredictable and Paws and Hands United Inc. cannot guarantee nor warrant that an animal being adopted may not become aggressive.

    I understand that animals can be destructive and may cause damage to property.

    I willingly accept that risk and will not seek reimbursement or legal action against Paws and Hands United Inc. for any damage.

    This contract releases Paws and Hands United Inc. from any and all liabilities.

    IMPORTANT: THIS DOCUMENT RELEIVES PAWS AND HANDS UNITED INC., ITS BOARD OF DIRECTORS AND

    VOLUNTEERS OR ANYONE ACTING ON BEHALF OF PAWS ANDHANDS UNITED INC. FROM LIABILITY RESULTING FROM PERSONAL INJURY, WRONGFUL DEATH AND PROPERTY DAMAGE CAUSED BY THIS ANIMAL.

    I have read this document, understand it and sign voluntarily.
