I hereby acknowledge the health of my child to be ready for vigorous activity at sports camp and assume all risks and responsibility for this. I further authorize the staff of Lockwood Sports Camp to act for me according to their best judgment in any emergency requiring medical attention if I cannot be reached. I hereby waive and release the camp, Fellowship Baptist Church and Lockwood School District, the directors and anyone connected with the camp from any and all liability for illness or injury incurred while at camp. I acknowledge that Fellowship Baptist Church and the camp do not provide insurance of any kind for the camp participants.
