Please take time to research your team's availability. Changes requested after this form is submitted will be required to follow the rescheduling policy. Please check the dates you are not available. Note: Requests based on school activities and soccer tournaments will be met. We will not guarantee requests for other reasons. There is the possibility of playing Sunday afternoons, but no earlier than 1:00pm. Please check both Saturday and Sunday if you need an entire weekend off. Six total conflict dates are allowed.
