If you are a Rotary club of Colorado Springs member who has attended a regular meeting of another Rotary club, local Rotaract club, Interact club, Rotary Community Corps, Rotary Fellowship, or another activity approved by the Rotry Club of Colorado Springs you can report your make-up activity by filling out and submitting the following form. Alternatively you can report your make-up activity in person at the registration table during our Friday's luncheon meetings. Fields marked with a red asterisk are mandatory.
