Announcement Authorization

Birthing In Korea would like permission to use a photo of you, your family, and or your baby in publications or on the new website.  Please fill in your name, email address and then click on the permission answer you would like to give.  We appreciate you allowing us to use photos that we have on file.  Please know that no photo will be used that show the delivery process or any nudity.  All photos chosen will be of the highest quality and content.

Birthing In Korea would also like your permission to add a birth announcement of your baby to our new website.  We recognize your baby may be a little bit older now, but we would still like to add announcements of all the babies birthed.  All birth announcements will contain first names only, date and time of birth, size and weight.

PHOTO RELEASE

By choosing "Yes, absolutely!" as your answer, you are agreeing to allow Birthing In Korea to use photos of you, your family, or your baby in marketing publications and on the website. 

By choosing "No, not at this time" as your answer, you are NOT allowing Birthing in Korea to use any photo of you, your family, or your baby in any marketing publications or on the website. 

BIRTH ANNOUNCEMENT RELEASE

By choosing "Yes, absolutely!" as your answer, you are agreeing to allow Birthing In Korea post a birth announcement using first names only, date and time of birth, size and weight.

By choosing "No, not at this time" as your answer, you are NOT allowing Birthing in Korea to publish a birth announcement.
