I acknowledge that my dog is at least 4 months of age and has all current shots (Rabies Distemper, Parvo, Bordatella, Influenza).

I understand that the same dog will need to start and finish the five-week session and I cannot intermix additional dogs that I may own or know.
I acknowledge that I am responsible for any harm that my dog may cause to other people, dogs, and/or property and I am solely responsible for the actions of my dog.
I acknowledge that Em-BARK Pet Care, and their contractors are not liable for any harm that may be inflicted upon my dog or me as a result of other people's dogs before, during or after the class.

I understand that the Canine ABC’s course that is offered by Em-BARK Pet Care only offers basic training information from the individuals of Em-BARK Pet Care. It is also understood that my $40.00 course fee is due before a spot will be saved, and that a full refund will only be given with 7 days notice of cancellation. Payment may be made below through PayPal and your spot will be immediately held. Checks and cash are also accepted and may be made payable and mailed along with this printed form to: Em-BARK Pet Care, P.O. Box 24037, Tempe, AZ 85285, and your spot will be held once payment has cleared.
