We need a copy of your latest veterinarian statement (showing rabies & distemper/parvo) at or before our first meeting. For board & train we also require the bordetella/kennel cough vaccination.

I agree that although the trainers will provide instruction and equipment for the training of my dog, I maintain control of my dog and I am responsible for the successful training, control and conduct of my dog at all times.

I am familiar with the training methods used at Dream Dogz, along with the tools, techniques, programs, and pricing. I will follow all instructions for the equipment and will not leave the equipment on my dog while unattended or overnight.

I am liable for any injury to persons or damage to property caused by my dog. I hold harmless and shall indemnify and defend the trainers from any and all injuries or damage caused by my dog.

If my dog is aggressive, I will use a muzzle during all training sessions and I will be responsible for avoiding potentially problematic situations. I understand there is an additional charge for working with aggressive dogs and aggression rehab.

I grant the trainers full permission to use photographs or video of this event for their purposes.

Because repetition is key to successful dog training, training packages must be completed within 4 months.

I understand I am responsible for scheduling personal training sessions with Dream Dogz and will be charged for appointments I miss without at least 12 hours notice.
