Once you (parent/Legal Guardian) have complete this form a church member will contact you to get a more detailed description of your child/children's needs and how we can best meet thise needs. Prior to the event, you will need to sign paperwork including a waiver of responsibility in order to participate. This is a service that will be provided by volunteers from First Free.
