Frontline Wellness Award Nomination

The 100 Club of Illinois is excited to be collecting nominations for our Frontline Wellness Awards! Each year we hear about the many sucess stories and inspirational testimonies of first responders who talk about a colleague and/or their agency who has gone above and beyond in the way of reducing the mental health stigma and/or providing programming and resources to better serve their members. We want to recognize them!

At our Frontline Convention, March 3 & 4 we will be honoring an individual and an agency who exemplify what it means to serve themselves and their members' mental health with our first annual Frontline Wellness Awards. It is our intention that through this recognition, we can showcase their work to thank them for their efforts while also inspire hope and dedication to others aspiring to serve their colleagues' mental health and wellbeing.

For the individual award, we are looking nominations of individuals who have made a monumental impact on their peers through peer to peer interactions, building wellness programming, advocating for mental health policies or support, or serving in some like capacity(ies) to serve their colleagues.

For the department award, we are looking for nominations of agencies who have built and/or implemented robust wellness programming to effectively serve their members in the way of peer support, policy development, training resources, family programming, positive culture, or other like capacities to create a strong wellness message and environment for their members.

To nominate an individual and/or agency, please complete this nomination form with all of the information requested in as much detail as possible to give us a full picture of the depths that your nominee has gone to create positive change. Share examples, data, stories, and/or details of the programming or service offered and list all contact information requested so we are able to follow up with appropriate members if selected.

Nominations are due no later than November 22, 2024. Awardees will be notified in early January and invited to attend the Frontline Convention to receive their award. To submit mulitple nomination, please complete this form for each nomination.
