  • Take A Soldier/Veteran/Active Duty Fishing - VOLUNTEER Registration With and Without Boats

  • You are completing the volunteer and boater signup form for a Take A Soldier/Veteran/Active Duty Fishing Event.  As a volunteer, you are certifying that you are able to assist the event coordinator with tasks that you have signed up for without compensation.  The event coordinator for Hagg Lake event is Jack Morby, morby517@aol.com (503-704-1842).  Expect notification of receipt of registration within 48 hours after submitting this form.   If you have been confirmed as a volunteer and you later cannot attend the Hagg Lake event, please contact Jack Morby as early as possible to cancel your participation.  Volunteers, Boat Captains with Boats, and Deck Hands are not required to be veterans or active duty to help with this event.

     

    If you have not received notification from TASF Portland Chapter within 48 hours after submitting registration form, it is likely your submission failed.  Contact Jack at morby517@aol.com (503) 704-1842 for further assistance.

  • What We Do With Your Information

    Personal information that is gathered on this form for Take a Soldier/Veteran/Active Duty Fishing, its partners, sponsors or any other affiliated organization will only be used for planning purposes for an event. Your information will not be sold, traded or otherwise given away without the express permission of the entrant. By filling out this form, you agree to allow your email address to be used for TASF newsletters and event emails. You may request your email to be removed at any time.
  • Hagg Lake event is located in Scoggins Park near Forest Grove Oregon off highway 47.

  • Additional Information

    In this area you will be asked a series of questions regarding the event you are signing up for. Please respond to all questions as completely as you are able. Failure to fill out the form completely and accurately may result in your entry form being deleted without notice.
  • If you are bringIing a boat for fishing, how many can you take? (Do not include yourself and deckhands; enter 0 if not applicable)

  • Please provide your boat license number. Include state of issuance. Enter "none" if not applicable.

  • Do you have Water Craft Liability policy or endorsement covering personal injury?

  • Event insurance required by Washington County Parks does not include guests or volunteers fishing from a boat.  Volunteers with boats assume the risk for all passengers on board their vessel.

  • If yes is checked above, please provide their name and have them complete a separate volunteer registration form and HHA.

  • Note: Please bring life preservers for the total number that you can take in your boat which includes guests, deckhands, and yourself. 

  • Note:  For Oregon events, Non-Residents or residents with water craft not requiring an Oregon boat license, must purchase an invasive species permit to use your water craft in any waters in the State of Oregon.  These permits are available through the Oregon State Marine Board web site and a variety of sporting good stores.

    • Some events may have T-shirts available for the boat captain and possibly other volunteers.  Shirt sizes are generally men sizes of small, medium, large, x-large, xx-large, and xxx-large.  The event coordinator will notify you if shirts are available.
  • Names of additional persons with you not including the boat helper and deckhand (First Name and Last Name only please)

  • Total number of people attending the event including yourself and deckhand/boat helper.

    In case of an Emergency situation and someone must be notified, please fill out the information below. Please give us the information to someone who is not attending the event with you. If you are part of a group that is coming and one person is in charge, please list their info here.

  • Please contact your event coordinator if you need additional fishing equipment for your guests.  Some event coordinators "may" have extra equipment to loan for the day.

  • Address is not required.  Please provide an address if you desire to receive event information by mail or desire to receive a thank you note for your volunteer efforts for the event you have signed up for.

  • Please specify any allergies, dietary restrictions, or special needs that you or any volunteer with you may have. Write "none" or N/A if not applicable.

  • Waiver of Liability/Hold Harmless Agreement (HHA)

    You agree to be bound by the Take a Soldier Fishing Hold Harmless Agreement. A copy of the HHA can be obtained from your Event Coordinator or directly from the TASF website at www.takeasoldierfishing.org. Take a Soldier Fishing will not sell, trade or otherwise give away any information that you provide on this form. The information gathered is simply for planning purposes for each event.
  • If no, go to www.takeasoldierfishing.org and click on Hold Harmless Agreement.  An agreement is required for every adult person participating (18 years and older).  Parents and guardians of children under 18 must list their names on the HHA and then sign as a representative for each of their family members.  Please bring the signed HHA with you to the event.  When you check-in at the event, the registration person will ask for your completed HHA or ask that you complete a new one (available in paper form at onsite checkin).

Once I submit my entry form I agree to be bound by any and all regulations set forth by TASF/TAVF, the Game and Wildlife division that controls the area, and any Local, State or Federal Laws. If you elect NO as your option, your entry form will be deleted and you will not be signed up for the event.
