You may complete one form for the entire family. Place the number of each meal you wish to purchase in the appropriate box. You may send in one check for all the meal and t-shirt orders for your family.

*All meals include lemonade, iced tea, or water. All lunches are $6.00 and dinners are $7.00. Meals will be cooked on site.

Dinner: Cheesy Chicken Spagetti, green beans, french bread, eclair cake

Lunch: Turkey or ham sandwich, chips, veggies and ranch dressing, brownie

T-Shirts
T-Shirt Order: Each shirt will have the name of the tournament on the front with names of all participants on the back. Each t-shirt is $13.00.
