LIABILITY WAIVER
I, the undersigned, do hereby voluntarily participate in the 2011 USTU-OHIO State Taekwondo and MAC-OHIO Goodwill-Open Regional Championship to be held on March 19, 2011 at Ohio Expo Center - Lausche Building. I assume full responsibility for any and all damages, injuries, deaths, or losses which I may sustain or incur while attending or participating. I hereby waive all claims against the tournament director, sponsors, officials, and individuals for injuries including their sole or partial negligence on their part which I may sustain. I fully understand that my application fee is non-refundable. I consent that any picture furnished by me or any pictures taken of me in connection with the tournament can be used for publicity, promotion, or television showing; and I waive all compensation in regard, thereto. I further acknowledge that this martial arts competition is dangerous and can result in injury. I am not entering this competition in reliance or any written or oral representations by the Ohio State Martial Arts Commission, Inc., or USAT regarding the applicable rules and qualifications for the judges and referees. I am in good health and physical condition to participate in this tournament. I further agree to surrender my pass upon request of the tournament official in charge. I have read and fully understand the above waiver.
Please note that all coaches must leave the competition floor immediately after the completion of his/her competitors’ match. The organizing committee reserves the right to suspend any and all coaching passes for failure to abide by the rules.
