SEC Bulk Order Form
Please use this form to make bulk orders for personal use, events, residence buildings, etc.

If you are in urgent need of safer sex supplies, please visit the UTSU Student Commons at 230 College Street and grab some supplies from our office (third floor, room 326) or the grab station in the hallway space just outside our office. If you need a large number of supplies, please use this form to palce a bulk order instead of grabbing everything from the grab station.

You will receive an automated email when your order is ready to be picked up 24hrs before your scheduled appointment (which you book at the end of this form).

Based on the Product Limits chart below, please give us a quantity next to the type of products in the following tables! (Note: quantities are per individual item unless otherwise noted). Keep in mind that depending on our stock availability, we may need to make some changes to your order. However, we will do our best to accommodate your request!
Please note that despite these limits, you are welcome to place multiple orders throughout the year! (Ideally, please leave a ~3-week window between large orders and please do not place multiple orders by different people for the same organization between that window)
