1. Baptisms are held preferably on the first Saturday of the month at 11.00 am or other dates in special cases of emergency or unforseen circumstances.

2. Baptism preparation is on the Thursday prior to the first Saturday of the month that is chosen at 7.30 pm in the church.

3. Please put the MOTHER's MAIDEN NAME in field assigned for the MIDDLE NAME.

Note on godparents: According to Canon Law, sponsors or godparents must "be a Catholic who has been confirmed and has already received the most holy sacrament of the Eucharist and who leads a life of faith in keeping with the function to be taken on;" and that a "baptized person who belongs to a non-Catholic ecclesial community is not to participate except together with a Catholic sponsor and then only as a witness of the baptism." (Can.874 §1.3; §2)

Please ensure that at least one of the principal sponsors/godparents is a Catholic in accordance with the Church law quoted above (Can.874 §1.3) . Additional godparents act as witnesses to the baptism (Can.874 §2).
