In consideration of my offer to participate being accepted, I intend to be legally bound, and do hereby, for myself, my heirs and executors, waive and release all rights and claims which may have or which may hereinafter accrue to me against HeartStone Foundation, City of Montgomery, Patricia L Baker, Bobbi Montgomery, Ed Steinebrey, Debbie Welling, Lauren DeFrain, Tom Welling, any subsidiary or political division thereof, their respective officers, agents, directors, representatives, other volunteers, beneficiaries, successor, assignees and sponsors for any and all damages or injuries which may be sustained and suffered by me in contention with my association with or entry/participation in the event as it is mentioned above.

If I should suffer injury or illness, I authorize officials of the ride to use their discretion to have me transported to a medical facility and I take full responsibility for this action.

I attest and certify that I am physically fit and sufficiently able to participate in this event.

I and any participating family members will wear a safety-certified, properly adjusted, and buckled bicycle helmet at all times while on the bike.

I grant full permission to any and all of the foregoing to use any photographs, videotapes, motion pictures, recording or any other records of this event for any purpose.

I have read the above release and understand that I am participating at this event at my own risk.

I have read, understood, and accept the agreement above. My submission of this form shall act as my legal signature. I acknowledge and extend this waiver agreement to other and/or minor members of my family who are also participating in the HeartStone Tour.

PLEASE NOTE:

* Other than the Family Ride, you are allowed to choose a different route (Advanced, Intermediate, or Novice) on the day of the ride. The above selections are primarily for planning purposes.

** For the Family Ride, one registration fee covers the entire family.

For the purposes of registration, a "FAMILY" is defined as one or more parents/step-parents/grandparents riding with one or more of their own children/grandchildren ages 14 years old or younger.

Children ages 15 and older are required to register as individuals.

If a family member is ALSO participating in one of the longer standard routes (Intermediate or Advanced) this person will need to register separately for these longer rides.

For example, let's say you are a family of four. If one parent plus both children wanted to do the Family Ride together, they would pay a single registration fee ($30) for the Family Ride. Then if the other parent wanted to do the Advanced Route, this person would pay their own, additional $30 registration fee for the longer ride. In contrast, if all four family members were doing the Family Ride together, then all four would register for the Family Ride and pay just one $30 registration fee.

To keep our overhead costs low, and thus maximize the amount of your contribution that can be passed on to HeartStone Foundation recipients, we have only a "simple" shopping cart feature set up with PayPal for the online registration process.

 What this means in practice is that we cannot provide separate areas for your registration fee vs. any optional donation you may choose to make. Also, your payment is processed in a "separate" window, not from within the registration form. But as long as you follow the steps below, everything will come through fine.

1) ALL RIDERS, please be sure to select the box below that says "Registration Fee".

2) If you ALSO wish to make a donation, please select one or more of the donation amounts below. Please note that you can select multiple donation boxes to produce a total donation of the desired amount (e.g. select both the $10 and $40 checkboxes if you wish to make a $50 donation).

3) After making your registration fee and optional donation selections (and double-checking them), press the "Submit" button below. At this point, you will have provided your registration information for the HeartStone Tour, but you still need to complete your payment.

4) For secure processing of your payment information, you next will be transferred to PayPal (in a new browser window). Please do not close the PayPal window or back out at this point! Your registration will not be complete if you do not finish the process of providing your payment through PayPal.

5) At the PayPal window, you can either pay via any major credit card (no PayPal account is needed). OR, if you already happen to have a PayPal account and want to use it, you can simply log onto PayPal and complete your payment this way. If you have not used PayPal before, please be assured that your payment is processed securely and confidentially. The HeartStone Foundation will receive notification of your payment, but all your payment details (e.g. credit card number) are kept private and are sent over a secure link.

6) Once you have successfully completed BOTH registration and payment, you will receive two confirmation emails:

a) A registration confirmation from HeartStone Tour
b) A payment confirmation from PayPal
