We request that the owners bring extra tasty food/treats to help entice your pet: treats, wet food, tuna (cats), baby food etc. Or anything they know the pet likes?

In the event that your pet does not eat, we have been instructed by our vet Dr. Dand to not give insulin. The reason for this is that the purpose of the insulin is to reduce the pet's blood glucose level because their body isn't doing this itself. If the pet doesn't eat, their level will already be lower and therefore administering the insulin will lower it even more and possibly to a dangerous level.

Proof of Up-To-Date Vaccinations are required PRIOR to stay. Please fax or email. ( FVRCP is mandatory at our facility.*

Uppon Arrival:

Please bring your cat in a crate to reception to check in at the front desk.
Bring medications or supplements in original bottles/packaging with you to the front desk.
We have plenty of cat toys available for our cat clients.
Do not bring, beds, blankets, stuffy toys, toys, dishes, collar or leash.
We ask that you take your crate with you after drop off as we do not have storage space.

All vaccinations: must be up to date for this visit.

Kittens require 3 sets of FVRCP (due in a year or more)

I understand that my pet will not be able to come into the facility without providing "UP TO DATE" vaccine records PRIOR to coming to Springbank Pet Resort.

In the event of an emergency and you are unable to be contacted, we will contact your Secondary Contacts on file. (We will verify these contacts at check in).

Medical Release:
IFirst Name Last Name , agree to the release of any pertinent medical information to Springbank pet Resort (collectively, "SPR"). I give them permission to contact my veterinarian for any information regarding vaccinations, previous injuries and/or medical issues etc.

In the event my pet(s) need medical attention, I allow Springbank Pet Hospital veterinary staff to attend to their needs. SPR will attempt to contact the owner or designated emergency contact in such an event. In the rare event that SPR cannot locate or contact you, the owner or the emergency contact, SPR will make decisions based on what is best for the health of your pet(s). SPR has permission to seek emergency medical assistance on my pet’s behalf. SPR is not financially responsible for any fees accrued in the process of obtaining medical attention and all fees are due to be paid at the end of the stay.

I, the undersigned, do hereby certify that I am the owner of the pet(s) or have permission from the owner to authorize this form and make decisions for the pet(s), I waive all claims for injuries, damages, parasites or loss he/she may sustain, due to participation in any and all activities associated with the use of this facility.

Photo Release:
Photos of pets are taken for your pet file and sometimes to post to social media or use in ads (No names or personal info is ever included)

I First Name Last Name, hereby grant to SPR, their staff and assignees thereof (collectively, “Photographer”) the perpetual, irrevocable, global and unrestricted right to use, reproduce and post (collectively, "Use") my pet’s picture, or likeness (collectively, "Image") in any media for publicity, art, advertising, trade, or for any other lawful purpose.

I understand that any Image may be edited, altered, or modified. I hereby waive any right to inspect or approve Photographer's Use of my pet’s image in any media.

I grant Photographer the right to market and sell copies of my Image. I also waive any right to royalties or other compensation related to Photographer's Use of my pet’s image.

I release Photographer and any legal representatives and assigns thereof from any claims in connection with the Use of my pet’s image. I intend for this agreement to bind all my heirs, emergency contacts, assignees, personal representatives, and members of my family.

I understand my name will never appear in any public setting, nor will my pet’s photo have any personal details visible.

Pick up (Unclaimed Pets):
Should my pet(s) be left unclaimed at SPR for more than 7 days past the original checkout date with no contact or alternative arrangements, my pet(s) will be surrendered to Rocky View County Bylaw and Animal Services. All outstanding debt will be sent for collection.

I understand that at the time of pick-up/discharge I will be responsible for all charges from services and items provided, and my pet shall not be released without proper payment.
