APPLICATION FORM

To apply, provide all the information requested below. At the end of this form, you will be prompted to submit your application along with a non-refundable processing fee of $30.

You are welcome to submit up to 6 images of your work. Provide details of each image in the space provided below.

Artists wishing to submit film or video clips in lieu of, or in addition to, still photos must first contact Margaret Shipman at gallery@brattleboromuseum.org or 802-257-0124 x 103.

Images must be in JPEG format at 72 dpi and no larger than 1024 pixels in any direction. The images themselves should not reveal the artist's identity, but each image should be titled in the following format: LasnameFirstinitial# (e.g., SmithJ2).

Please complete the following for each image submitted. Note: For "mixed media" works, please provide some detail of the media utilized.
