  • How did you find this site?*

  • How will you pick up your puppy?*

  • Have you researched the breed and know the requirements for grooming and health care?  (Grooming is extensive and costly!  These are not low maintenance dogs!)*
  • Please explain your current family situation:*

  • If you have children, are any of them fearful of dogs? Do your children have experience around young puppies. Children must be taught to be gentle and respectful of a young puppy and how fragile they can be.*
  • Explain your housing situation:*

  • Do you agree to spay or neuter?  Females by 9 months, males by 6 months of age*
  • Do you have a Website?
  • Do you understand the requirements of the breed you are inquiring about? Dogs are expensive and require regular grooming and vet care. Puppies require followup vaccinations and monthly deworming and heart worm prevention. Grooming can cost from $50-120 per visit every 6-8 weeks. You also need to know the genetic tendencies and issues each breed can have. This is a major decision and should NOT be an impulse buy. Puppies are like children--they are expensive and will live with you a long time and you must love and care for them no matter what and are NOT disposable!*
  • I understand that puppies will probably:  jump, chew/bite, lick, potty accidents in your floor, crying at length at new home first few nights, bark when left alone, etc.  I also understand that it can take months to train a puppy and will take A LOT of time and devotion,  These are BABIES and do not come fully trained.  We can't emphasize enough, how much time it takes to properly train and raise a puppy.*
  • Will you be able to continue premium food and the supplements we recommend?  And provide vet care for vaccines, routine vet care, heartworm prevention, flea/tick prevention, etc  .*
  • Can you provide pet health insurance?  Did you know a broken leg surgery can run into $5000 or a swallowed sock or toy the same?  We want to know our pup can receive all the care it deserves and that vet care doesn't get skimped on because of finances.  Most people don't know about canine pet insurance that it is affordable, plans from $40-80 a month depending on coverage you select.  Some plans even include first year spay/neuter.  It's definitely something to consider when vet care is so very expensive.*
  • Do you agree to attend puppy classes?  AFTER 14wks old.  Even if you have raised a puppy before, it is a great idea to start out on the right foot by taking them to classes.  They benefit from the socialization and  you learn how to train them to be the wonderful dog they can and want to be.  The puppy will need to have FOUR puppy boosters before they can safely be around other dogs.*
  • Are you a 1st Time Pet Owner?*
  • Do you have young children (under the age of 10) in the home?*
  • Thank you for taking the time to complete this form. Please understand not all applications will be accepted in the best interest of the puppy. This information will NOT be shared. Gina will follow up asap.

  • First Time Pet Owner Questionaire

  • Do you realize it takes months to potty to train a puppy? That there can be setbacks and take tremendous amounts of time and dedication.  Puppies take enormous amounts of time and are a HUGE responsibility.
  • Do you realize a young puppy does not come to you leash trained, potty trained or crate trained?  All the training will be up to you.  Puppies will jump up on children and will do lots of things that are inappropriate and will need to be taught not to do these things
  • The list below are things puppies can do that are normal.  Please agree that you understand this is normal puppy behavior: Chewing on your hands, your shoes, your furniture, etc.  Potty accidents on your floors, on their bed or in their crate.  Barking possibly all night till adjusted to new home  Licking their own bottoms and  sometimes eating their own poo.  Going potty in the house just after coming in from outside. Tearing up anything left on the floor (paper towels, Kleenex, mail, books, etc.}
  • Do you agree to take puppy classes as soon as puppy vaccinations are done?
  • Do you agree to contact a professional trainer to help in case you have difficulty with training.
  • In the event that you feel you need to re home the puppy, do you agree to contact us first?
  • Do you realize non-shedding breeds like ours require regular grooming every 6-8 weeks and they need their ears hairs pulled, anal glands drained, nails cut and potty trails trimmed?  Have you called to check the price for a grooming?  How much is it in your area?
  • Do you understand that male and female puppies will hump things and that this is anormal behavior that will eventually go away
  • Did you know that 90% of behavior problems in puppies are the owners fault?
  • Do you know that If you hold the puppy non stop for the first day or two, and not give the puppy alone time while you are home or gone this will cause severe separation anxiety that is very difficult to break?
  • Do you understand that you can NOT crate a puppy all night and all day while you are at work?  This can absolutely cause a dog to go crazy and can definitely cause problems with training.
  • Do you know that you can not give the puppy free reign of the house?  If you can not watch the puppy 100% of the time, it needs to be in an exercise pen or confined area.
  • Have you puppy proofed your house inside and out?  Problems usually involve wires, stairwells, chemicals, food, mulch, eating rocks or poisonous plants trash, etc.
  • Do you plan to buy an exercise pen for safe indoor play when you are busy? This is a life saver!  Also same goes for outside.  Do you have an exercise plan for safe outdoor playtime?
  • Is all of your family on board with this decision to get a new puppy?
  • Have you decided who will baby sit for you when on vacation or away?  Boarding at the vet means just being crated all day with no training or interaction.
  • Did you know that Parvo can be FATAL and is a highly contagious dog disease and all puppies are NOT protected until they have had FOUR rounds of the parvo booster?  Parvo lives in the dirt or on surfaces for YEARS and can be in the grass or at dog parks or rest areas or even in your back yard.  It is on the floor at the vets offices and possibly on the bottom of people's feet at the grocery store, petsmart and even can be at the airport.
  • When you get a new puppy you should not take it anywhere until it's had all FOUR puppy shots.  No park, no walking in public, no petsmart, no friendshouses, just stay home and be safe.  Do you understand this?
  • Do you realize what vet care costs are in your area?  A broken leg can cost thousands.  Diarrhea can run into hundreds of dollars in tests and medications.  All vet bills are your responsibility.  We always recommend everyone buys pet health insurance.
  • Families with young Children Questionaire

  • Are any of your children afraid of puppies/dogs or have had a bad experience withanother dog?  If so, please explain:
  • Have your children been raised around dogs or been around young puppies? How did they act?
  • How would you describe the energy level in your home?
  • Will you agree to puppy classes when pup is done with puppy shots and take your children with you to the classes?
  • Do you agree to hire a personal trainer at the first sign of behavioral issues?
  • One of our main concerns placing a puppy in a home with young children is the relationship that dogs can develop with toddlers.  It can be a wonderful loving relationship or the pup can become traumatized by hyper children wanting to play rough and chase.  If the pup gets injured while playing, the pup will learn how to defend itself and start to nip, growl or eventually bite.  This behavior can continually get worse over time.  The sad result is then trying to re-home the pup that already has a history with children.  Also the possibility of being placed in a shelter or found just any home.  We ask that you agree to contact us immediately if you have such issues and we will either help you overcome them or agree to take the puppy back.   Each case is different but most of the time we can purchase the puppy back and find them a great home.   Do you agree to contact us with ANY behavioral issues:
