Step 1: Complete the W-9 Form and then upload a copy belowStep 2: Complete the Child Clearance Online Form
If you already have a valid PA Child Clearance (obtained after July 1, 2015), simply upload a copy below. If not, Create an individual account and a username. You will get a confirmation email to create a clearance application. After your clearance application is approved by Pennsylvania (approximently 6 weeks), upload a copy of the approved clearance document below. You are entitled to one (1) night in a complimentary hotel room on the night between your Future 500 work dates. If you are traveling from a longer distance, please see the hotel room guidelines below.
If you are working the first 2 days of camp:
1. Coaches that are flying into Philadelphia or have a trip of longer than 8 hours can receive a complimentary hotel room the night before the 1st day of camp. We will start at approximently 1:15pm the first day of camp.
2. Coaches that are flying out of Philadelphia or have a trip of longer than 4.5 hours may receive a complimentary hotel room the night of day 2. We end camp by 8:30pm on Day 2.
If you are working the last 2 days of camp:
1. Coaches who are flying into Philadelphia or have a trip longer than 3 hours can receive a complimentary hotel room the night before Day 3 of camp. We start at approximently 7:45am on Day 3.
2. Camp will end by 1:30pm on Day 4 and all coaches are expected to depart at that time.

Nothing contained herein or in any other document executed in connection with Future 500 ID Camps, shall be construed to create an employer-employee relationship. Contractor acknowledges that he/she is an independent contractor and not an employee of Future 500 ID Camps. Contractor acknowledges that Future 500 ID Camps will not deduct or withhold any taxes, FICA or other deductions that we are legally required to make from the pay of regular employees. As an independent contractor, you will not be entitled to any fringe benefits, such as unemployment insurance, worker’s compensation, medical insurance, pension plans or other such benefits that would be offered to regular employees. By checking the box in the college commitment form, you acknowledge that you have read, understand and accept the terms/conditions.
