SOZO Ministry Application

Please complete this application as the first step to obtaining an appointment for Sozo prayer ministry.

Your name will be added to a waiting list in our Sozo ministry. Your information will be kept confidential by the administrator and administrative staff overseeing this ministry. A copy of your application will be given to the leader of the Sozo team you are assigned to. This is for purposes of prayer and preparation of the ministry team.

If you feel you are in need of immediate ministry attention, you should call your home church or our church at 719-315-4769.

The Sozo ministry does not provide home visitation, family or personal intervention or crisis ministry. At this time, we do not provide any on-going support group ministry. This ministry depends upon trained and equipped volunteers and will strive to coordinate appointments around their availability. Please allow a week for someone from our team to get back with you, usually via email. Donations to help support the ministry are appreciated.

Failure to keep an assigned appointment will result in loss of priority in scheduling a new appointment. if for any reason you will not be able to keep your appointment, you must make every effort to call 719-315-4769 or email royalgorgevineyard@gmail.com PRIOR to your appointment to let us know you must cancel and reschedule.

Once your application has been received, we have a team that will be praying for you. Within a few weeks, you'll receive an email that includes a web link to schedule your time. Currently, we are offering times on Tuesdays, Wednesdays and Fridays. (Occasionally, we receive an application that we don't accept for various reasons. If this is the case, someone will let you know, and refer you to other resources).

I acknowledge that team members from Royal Gorge Vineyard Church (RGV) have voluntarily agreed to pray for me. I understand that this session is not a professional counseling meeting and that none of the team members are licensed counselors. I understand that these team members are, to the best of their ability, doing what they can to help me achieve more freedom in my life.

I understand that RGV is a nonprofit church organization that makes no charge for its services. I further state that I have voluntarily sought assistance of my own initiative and that I am under no obligation to accept or reject any of the advice or help that I might receive from the team members of this ministry.

Our team members offer Biblical spiritual services to anyone who desires them regardless of ability to pay. Although there is no charge for our services, all efforts to build this ministry support and train our team members are paid directly from donations of those receiving these services. We therefore have a suggested donation of $50.00 or more per visit. Your contributions to this ministry are greatly appreciated and we can provide you with a tax deductible receipt.

I understand that if I receive ministry from RGV, the teams are committed to respecting the disclosed information, but not to complete confidentiality. The information, as needed, may be shared with other leaders of sozo ministry so as to further your total healing process. This may include future meetings with spiritual mentors in the church to set appropriate boundaries for your personal and spiritual growth.

I agree to hold RGV and its team members free from any and all liability, loss or damage of any kind that may arise as a result of assistance which I have received or from my involvement with RGV.
