The National Center for Complex Health and Social Needs (National Center), an initiative of the Camden Coalition, published a “complex care startup toolkit” with support from the Robert Wood Johnson Foundation for organizations that are developing and implementing new complex care programs. The toolkit contains resources, guides, and sample materials to help organizations start and grow programs that support individuals with complex health and social needs.
