Team Registration (Providing Your Own Player Cards)

Use this form only if your club is providing your own player cards from USSSA to register a team for league play with SAI. All carded personnel (coaches/managers) must also complete and submit the coach requirements. Directions and links can be found at www.saisoccer.org under Coach Forms. Please also complete the Schedule Request form when it is available for this season. That link will be emailed to you when it is ready.
