A request due to Health reasons
Yes, you will need to supply a supporting document (such as a medical note). Please ensure that the document gives as much detail as possible. You must also make sure that your supporting statement ("Details of circumstances", above) provides enough detail for the Mitigating Circumstances Committee to make its decision.
There are some exceptions.
If you:
- have a pre-existing medical condition (chonic or other long-impact)
and
- you have already supplied evidence of this condition for an extension request made in the last 12 months
and
- your condition has not significantly changed
you do not need to supply a new medical note. However, you may still supply other supporting documentation which helps us understand the impact your condition has had on your ability to complete assessment(s) listed above.
A request due to Assessed Learning Needs
No, you do not need to supply supporting documents as long as your Learning Need has been assessed and registered with the NTC Learning Support Co-ordinator.
If you have not had a Learning Need assessed, you cannot apply for an extension on these grounds. You must get your suspected Learning Need (physical and/or mental) assessed - the Learning Support Co-ordinator will be able to assist you to get this assessment. You must select "Other Reasons" or "Health Reasons", whichever is appropriate to your situation.
If you have an assessed Learning Need but have not registered this with the college, please contact the Learning Support Co-ordinator as soon as possible. You may be able to apply for an extension at this stage.
A request due to Other reasons
We strongly recommend that you supply a supporting document, although this is not technically required. A statement from your course tutor, the Chaplain, your Pastoral Care Group leader or another third party may be useful. You should also give us copies of anything else which may support your request . Please ensure that any document gives as much detail as possible.
