  • The Civic Service Institute at NAU is pleased to announce that nominations are being accepted for the 4th Annual GEMS Awards. Please nominate a deserving VOLUNTEER—individual, civic group, nonprofit, or business for one of the awards. Nominators are limited to nominating one person or group per category.

  • Event Description

    The Civic Service Institute’s GEMS Flagstaff Citywide Volunteer Luncheon, sponsored by Findlay Auto Group, will be held Thursday, April 23, 2020, at the DoubleTree Hotel, 1175 W Rte 66, Flagstaff, AZ 86001. Doors will open at 11:00 am for hors d'oeuvres with lunch being served starting at 11:45. GEMS recognizes the “gems” we have in our community; those volunteers who go above and beyond and make our community the greatest place to live.

    Nominations for volunteer awards are currently open under the following categories:

    • Youth (up to age 17)
    • Young Adult (ages 18-24)
    • Adult (ages 25-54)
    • Senior (age 55 and over)
    • Family (those volunteering as a family together)
    • National Service (Current AmeriCorps, VISTA, or Senior Corps member)
    • International Service (local folks volunteering outside the US)
    • Civic Organization (those groups volunteering for the benefit of the community)
    • Business (those who encourage their employees to volunteer or who commit to volunteer as a business)
    • Nonprofit (organizations which utilize volunteers to their fullest)

    *sponsorships for a table include 2 seats for the sponsors and 6 seats for the organization of their choosing.*

    If you prefer to send a check you may do so, please include a list of those sitting at your table, their entree option, and list any allergies. Checks can be written out to GEMS Civic Service Institute and sent to P.O Box 5063 Flagstaff, AZ 86011, you have until April 16 to send checks.

  • How to Nominate

    Nominating a volunteer for an award is easy; simply complete the form in the coming pages. What you might want to have handy prior to entering the nomination form is the volunteer’s name, address, phone number, email address, and photo (pdf). After entering that information, you will have the opportunity to write an essay (350 words or less) about why the volunteer should be the award winner. You may nominate individuals, organizations, and businesses in the following categories: Youth, Young Adult, Adult, Senior, Family, National Service, International Service, Civic Group, Nonprofit Organization, and Business.
  • Nomination Guidelines and Criteria

  • Who is Eligible?

    • Any individual, civic group, nonprofit organization, business, or international volunteers who provide an exceptional level of support, activity, or commitment and/or leadership to your organization.
    • Nominees must have been engaged in volunteer activities for a minimum of nine (9) months within the community, with the exception of national service and young adult.
    • Nominees are eligible in ONE category only.

    Who is NOT Eligible?

    • Previous GEMS winners.
    • Individuals involved in “court mandated community service”.

    ALL NOMINATIONS MUST BE SUBMITTED BY FRIDAY, FEBRUARY 7, 2020. ONLY ONE NOMINEE MAY BE SUBMITTED PER CATEGORY PER NOMINATOR. Nomination forms received after February 7th MAY be considered.

    If the nominee is a group/team or business the name of one contact person must be provided. The entire group/team or business is more than welcome to attend, however if all cannot attend that individual will be the person who will attend the ceremony and accept the award on behalf of the group or business.

    • All information requested must be provided in order for the nomination form to be considered complete. Incomplete forms will not be processed.
    • Nomination forms will only be accepted online, no other supporting documentation sent to us will be considered.
