Dogs are 1st come 1st serve basis between approved adopters, please what until you can come to apply. We are NOT an 8-5 shelter we show by APPOINTMENT only.. PLEASE, PLEASE, PLEASE, DO NOT FILL THIS APPLICATION OUT UNLESS YOU ARE READY TO ADOPT, CAN AFFORD THE DOG AND DRIVE TO PRAGUE WITHIN A DAY OR TWO. So many are filling apps out as time fillers I suppose, please do not do that. It is a total waste of my valuable time.
Ok Save A Dog Rescue is an all-volunteer, non-profit, animal welfare organization dedicated to the rescue and placement of stray, neglected and abused animals. Ok Save A Dog Society is completely funded through fundraising efforts and public donations. We use very few foster homes and know our dogs intimately, we will only place them where we feel they will thrive and we retain the right to refuse adoption to anyone for any reason. ALL dogs receive complete medical care, which includes worming, vaccinations, spaying/neutering HW test and treatment if needed and microchipping prior to their adoption.
This questionnaire is designed to help Ok Save A Dog Society and yourself determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. Ok Save A Dog Society reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions! IF you are denied it does not necessarily mean we think you are a bad home it means your home is not right for this particular dog.
