Member Sponsor Form

Applicants must be sponsored by two MTJA members in good standing. Sponsors must be MTJA members for at least a year.

Members may sponsor two Associate members each year and any number of Actives. Sponsors must submit this form for the potential member application to be reviewed.

It is the applicant’s responsibility to ensure sponsor forms are submitted in a timely manner. MTJA will not solicit forms.

Please be candid. This information will be reviewed only by the Membership Committee and Board of Directors.

Current MTJA members may only sponsor two Associates per year, and may sponsor any number of Actives. Any Sponsor Form submitted without comments, not fully completed, not legible, or not signed, will be returned and (if it impedes review schedule) may jeopardize applicant’s eligibility.
