We request that in May you notify the Leader of relevant diseases or infections that are current at that time - that could affect other campers.
If it becomes necessary for any of the above named young persons to receive medical treatment then a family member will be sought, however a parent cannot be contacted to assist this, I hereby give my general consent to any necessary medical treatment and authoriset the Leader in charge to sign any document required by the hospital authorities.
