Please complete and sign the following waivers for your daughter(s) who will be attending camp this summer.

Horse Related Activity Permission Form, Waiver and Release of Liability

With this waiver, I accept notice of the provisions of the Equine Activity Liability Act          3.1-796.130 through 3.0-796.133 of the Code of Virginiad, 1950, as amended, which state in part:  "(I) the propensity of an equine to behave in dangerous ways which may result in injury to the participant; (II) the inability to predict an equine's reaction to sound, movements, objects, persons or animals; and (III) the hazards of surface or subsurface conditions."

I/We the undersigned do give my/our child permission to participate in all horse related and equine activities.  I/We do hereby assume the risk of participation, by ourselves of the camper listed above in any and all equine activity, or in any presence in or around the horse stables, in association with Camp Strawderman, Inc.  I/We voluntarily assume all risk of loss, damage, illness, or injury that may be sustained while so engaged or as a result thereof.  I/We agree to hold harmless, release and indemnify Camp Strawderman, Inc., and its officers, employees, and agents in any location where horse-related activities are conducted, horses and/or property are used, of and from all claims, which may hereafter accrue on account of any injury, loss, or damage.  The person executing this release acknowledges that there is a valid consideration in executing this release.  The invalidity of any statement or waiver of rights above under local, state, or federal law does not invalidate any other statement or waiver of rights herein.  

The waiver shall remain valid for all camp attendance for this camp session and future camp sessions, unless expressly revoked in writing, with receipt acknowledged by a representative of Camp Strawderman, Inc.  By signing this Waiver and Release, I understand that I am giving up (waiving and releasing) any action I may have on my behalf, or on the camper's behalf, by virtue of participation in horse related or equine activities including lessons, jumping, cross country jumping, trail rides and horse shows, and I do so knowingly and voluntarily.

I have reviewed and understand the camp program and schedule, and I understand there are risks involved with all camp activities.  I hereby waive and release any liability which may arise due to my child's participation in the camp activities listed above and otherwise, and do indemnify and hold Camp Strawderan, Inc., its agents, employees and representatives harmless for the same, and do give my child permission to participate in all appropriate programs.  I have discussed any limitations, including medical conditions or needs, with the camp's administration and nursing staff prior to check-in at camp.

COVID:  I understand and agree that Camp Strawderman, Inc. (“Strawderman”) has taken all reasonable precautions to protect participants, guests, and staff from exposure to COVID-19. I agree that by my child attending Strawderman, I will fully comply with all such measures and, my child will fully comply with all such measures, and all persons visiting or accompanying my child at Strawderman will comply with such measures. I further acknowledge that my child’s attendance may expose me and/or my child and/or a guest of my child to the risk of exposure to COVID-19 in spite of any and all safety measures and I assume any such risk that may arise therefrom.  By signing this Waiver/Release, I hereby release, indemnify and hold harmless Camp Strawderman, Inc., its agents, employees and representatives, and volunteers (“released parties”) from and against any and all claims, demands, actions, complaints, suits or other forms of liability that any of them may sustain arising out of (a) my child’s contact with, or attendance at, Strawderman (including accompanying or visiting persons with or to my child at Strawderman), (b) a failure of my child or any person associated with my child to comply with the measures imposed by the released parties, (c) a failure to comply with local, state, and federal laws and policies, procedures; or (d) arising out of any damage or injury caused by myself or my child, including, but not limited to any and all related costs, attorney fees, liabilities, settlements, and/or judgments.  

Photo Release

I give permission for Camp Strawderman to use a picture or pictures (not a name) of my daughter(s) either as an individual or within a group for camp publications which include the camp website, a newsletter, the camp video/CD/DVD, and the Strawderman Horn.
