Welcome to CITREP+ E-Application Form. Before you proceed, there are a few documents that are required to process your application. Kindly see below and prepare the relevant documents before continuing. Please ensure all information filled in is correct before submitting.

 

 

1.   For Students:

• Proof of matriculation
• Recommendation by the PSEI (Form 1)
   

2.   For Students below 18 years old:

• Proof of matriculation
• Recommendation by the PSEI (Form 1)
• Parent/ Guardian’s consent Form (Form 1A)
   

3.   For NSF (Army):

• Documentation Proof of Enlistment and Operationally Ready Date (ORD)
• Copy of trainee’s SAF 11B card (Army Card)
   

4.   For NSF below 18 years old:

• Documentation Proof of Enlistment and Operationally Ready Date (ORD)
• Copy of trainee’s SAF 11B card (Army Card)
• Parent/ Guardian’s consent Form (Form 1A)
   

5.   For SME:

• Company’s Declaration of SME Status for Funding Support (Form 2)
