This form is to be used by staff to request or change Salary Packaging details for Sports Centre membership.

All fields and sections marked with an asterisk (*) are mandatory.
It is recommended that staff seek independent financial advice before salary packaging.
Once this form has been completed it must be printed, signed and sent to the relevant provider or area for initial approval and processing. Once approved, the relevant provider or area will advise payroll to complete processing.

1.6 - Cancellation of Membership Direct
Debit memberships may be cancelled at any time (after the completion of minimum 8 fortnightly payments) with the provision of a written cancellation form with 14 days notice. Email, over the phone and facsimile, will not be accepted.
2.3 Salary Packaged Memberships
Requires a Membership Application and an HR32 form to be completed. Payments are deducted pre-tax from your University salary. Salary packaged fortnightly membership payments are ongoing until you advise MU Sport in writing that you wish to cancel your membership. A period of 14 days written notice must be given to MU Sport. A suspension cannot be used with this membership type.
