    PARKING RULES AND REGULATIONS:
    (By submitting this form, you agree to all the following)

    PAYMENTS
    BOH Employees: Automatic payment deductions will be set up via payroll.  Please complete this parking application form to initiate the parking and payroll process.

    No cash payments will be accepted. There will be no substitutions, vacation credits, or prorated charges.  

    ADMINISTRATIVE FEE
    Please refer to the Security Access Card (SAC) Request Form for further information.

    SECURITY ACCESS CARDS (SAC)
    Employee understands that their assigned SAC badges will allow the registered card holder access to their designated location within the set time zones specific to the location. SAC badges are non-transferable and therefore must be returned immediately upon cancellation. SAC badges are to be used by the registered monthly parkers only and may not be "lent" to anyone else for their use. Doing so will result in the immediate loss of parking privileges.

    LICENSE PLATE RECOGNITION (LPR)
    All registered vehicles license plates are entered into the License Plate Recognition (LPR) system.  Registered license plates are a form of a monthly parking credential in which the LPR system recognizes and allows entry and exit of the parking garage.  If the vehicle you are operating has not been registered (i.e., new vehicle, rental vehicle, etc.), please scan your SAC badge as a secondary parking credential to access the parking garage and notify/update Elite Parking Services of your vehicle situation at https://eliteparking.net/boh-hok.

    PASSBACK 
    All registered vehicle information and SAC badges are programmed to allow only one vehicle to be in the garage at one time. The cycle begins when entering the garage by LPR or scanning your SAC badge only once while the arm is completely down. The cycle is completed when exiting, again by LPR or scanning your SAC badge only once when the arm is completely down. If an access card is used out of sequence, such as multiple attempts to enter by LPR and scanning your SAC badge twice, it causes a "pass back", which is considered a misuse of your parking credential. If your parking account activity has repetitive pass backs, you will be given a warning and may lead to termination of this contract.

    REPLACEMENT SECURITY ACCESS CARDS
    For all lost, stolen, or damaged access cards, please notify Elite Parking and BOH Security immediately. Please reach out to Corporate Facilities for a replacement card refer to the Security Access Card (SAC) Request Form for further information.

    FORGOTTEN SECURITY ACCESS CARDS
    If you forget your SAC badge and the LPR system does not recognize the vehicle you are operating, entry and/or exit into the parking facility will require the registered Parker to pull a parking ticket and pay the general parking rate with a debit/credit card or validation upon exiting the garage. The amount paid is non-refundable. As a reminder, the registered Parker is required to have their SAC badge to validate use of the parking facility. 

    RESERVED PARKING STALLS/AREAS
    Employee understands and agrees that monthly parking is for one vehicle only and that space may not be loaned, gifted, or sublet. Any parking stall designated as "Reserved" is off limits to anyone but the assigned owner of the stall. Employee must park in an assigned space (if applicable) and may only occupy one space at a time. Vehicles parked outside of their designated area/stall in Bank of Hawai`i customer parking, handicap stalls (without displaying the required permit/tag), "No Parking" zones, "Reserved" stalls (without a valid permit), and those occupying more than one stall or blocking ingress/egress may be subject to tow at the owner's expense (in accordance with Section 290-11 of the Hawaii Revised Statutes) and/or consequently have their parking privileges revoked without consideration to reinstate monthly parking.

    VEHICLE INFORMATION
    It is required to always keep your vehicle information current. Your license plate(s) are registered to your account and allow entry and exit into the parking facility. Should any of your vehicle information change, you must notify parking management and fill out an updated information form at https://eliteparking.net/boh-hok.

    PARKING CANCELLATIONS
    All parking cancellations must be submitted on or before the end of the month prior to cancellation. Please fill out the cancellation form at https://eliteparking.net/boh-hok. SAC badges must be returned to BOH Security at or before the end of the month. Parking fees will not be prorated for partial months.

    SAFETY PRECAUTIONS
    For safety reasons, the speed limit at all Bank of Hawai`i facilities and lots is 5mph. Please observe this speed limit and be mindful of other vehicles when backing out of stalls or maneuvering through the lot or facility. Please do not leave valuables in your vehicle and lock all doors before leaving. Please report any suspicious activity to Bank of Hawai`i Corporate Security by calling 694-1911.

    EMPLOYEE UNDERSTANDING
    Elite Parking Services has been contracted to manage this Bank of Hawai`i parking garage. All parking registrations, cancellations, updates and validation orders must be directed to https://eliteparking.net/boh-hok or email us at boh@eliteparking.net with any questions/concerns.

    Employee understands and agrees that he or she may be responsible for the cost incurred to clean up his or her stall if it is determined that Employee’s vehicle is leaking or staining the lot or facility.

    As a condition of parking in Bank of Hawai`i lots or facilities, Employee and vehicle owner assume all risk, loss, or damage while parking or maneuvering through and about the lot or facility and neither Bank of Hawai`i nor Elite Parking Services shall be liable for any loss or damage arising out of, or caused by theft, mischief, vandalism, collision, fire any act of God (such as but not limited to earthquake or flood), or any other such casualty or incident on or about the subject property.

    Bank of Hawai`i reserves the right to reassign stalls as needed by providing Employee with notice and assignment of a new stall. If this becomes necessary, Employee shall have five (5) working days to accept the new stall. If an Employee rejects the new assigned stall, this parking agreement will be terminated effective immediately and a full refund for the then current month will be given via payroll.

    Bank of Hawai`i reserves the right to cancel and/or terminate this agreement or to change fees/rates associated with parking at any time by giving Employee thirty (30) days prior written notice. Failure to do so may result in payroll deductions after the effective date of termination.

    PART-TIME PARKING ACCESS - CURRENTLY HAVE MONTHLY PARKING AT A DIFFERENT BOH LOCATION

    For all that already have monthly parking at a different BOH location and would like to add part-time monthly parking at HOK, the following rules apply:

    • Adding the part-time monthly parking option will not be included in my current payroll deduction.
    • Elite Parking will invoice me directly on a monthly basis for my additional monthly parking. I will have the option to pay for my part-time monthly parking by check, online or set up automatic monthly payments directly with Elite Parking.
    • I am only allowed to use my part-time monthly parking access eight (8) occurrences per month. Should I exceed the eight (8) occurrences, I will be required to pull a ticket, obtain a "PT HOK" parking validation from Security, and pay the $2.25 flat rate upon exit.
    • This part-time monthly parking does not allow License Plate Recognition (LPR) programming due to limited parking access at this location.
    • Failure to comply with the aforementioned conditions and the standard parking rules and regulations may subject my part-time monthly parking privileges to be revoked.

The management shall not be responsible for fire, theft, damage, or loss to said automobile or any other article left in it.     • Part-time monthly parking is $17.50 + GET per month.
    • Elite Parking will invoice me directly on a monthly basis for my additional parking. I will have the option to pay for my part-time monthly parking online or set up automatic recurring monthly payments directly with Elite Parking.
    • I am only allowed to use my part-time monthly parking access up to eight (8) occurrences per month. I will automatically be billed for full-time ($35 + GET) should I exceed the allowed eight (8) occurrences.
    • This part-time monthly parking does not allow License Plate Recognition (LPR) programming due to limited parking access at this location.
    • Failure to comply with the aforementioned conditions and the standard parking rules and regulations may subject my part-time monthly parking privileges to be revoked.

  • I authorize Bank of Hawaii, People Services Department, to deduct the parking fee checked below from my payroll each month on a Pre-Tax basis. The parking fee will be evenly divided between the first and second pay period of each month.  Upon receipt and approval of this form, parking will begin effective the date indicated.

  • For Pre-Tax deductions, I understand that:

    • My election will remain in form until I submit a new election form (to change information or terminate election).  It is my responsibility to notify Elite Parking Services should my status, shift, or work location change as these changes may impact the amount of my parking deduction.
    •  I may not participate in the Bus Pass Voucher Program nor the Bicycle Commuting Reimbursement Program for the same time period.
    • My election will automatically terminate if I cease to receive compensation from the Company that is at least equal to the amount covered by this election.
    • I am ineligible to enroll in the Transportation Expense Plan (Transportation Flexible Spending Account).
    • Pre-Tax salary reductions pursuant to a Pre-Tax election reduce my compensation for Social Security tax purposes.  This means that my future Social Security benefits could be decreased because of the decreased amount of compensation that is considered for Social Security purposes.
    • After the start of any month, the amount deducted for that month's parking expense will not be subject to refund or cash out.

  • ORDERING VALIDATIONS

    GENERAL INFORMATION

    • All validations are sold in sheets of 40
    • Sheets of validations are individually numbered
    • No returns or exchanges of unused validations allowed
    • Only authorized individuals are allowed to order validations. If you are not authorized, we will have to confirm with your company prior to order being completed

    VALIDATION FULFILLMENT

    • Delivery of validations are only fulfilled on THURSDAYS during normal office hours
    • Validation orders must be received by 12PM THE DAY PRIOR (WEDNESDAY), or they will not be ready until the next fulfillment day
    • Validations are made to order each time

    PAYMENTS

    • Only CHECK or ECH payments will be accepted
    • Payment will be required upon delivery

    INVOICING

    • If you require an invoice, please be sure to notate that in your order.
    • A receipt will be provided
