The above pet (prior to adoption) has been examined by a vet and considered to be in good health at the time of adoption. To the best of our knowledge this pet will be a suitable match with the adopter and all pertinent information about this animal's past has been explained to the new owner. If for any reason said pet becomes ill or needs medical attention within five (5) days of adoption for health problems (other than accidents), PAWS must be notified first by calling (440) 442- PAWS (7297) or the phone number of the foster parent listed above on this contract. PAWS will advise where the pet should be taken for medical care. PAWS will only pay medical expenses if PAWS is notified first, and if said pet is taken to a PAWS vet within 24 hours after making contact with a PAWS representative. PAWS is not responsible for any household or personal damage done by said pet. PAWS advises that all new pets be restricted for the first few days until they learn their new routine. PAWS will not be responsible for medical attention for said pet beyond the 5th day of adoption.
PAWS reserves the right to make follow-up visits or phone calls concerning the animal's health and well being and hopes the new adopter understands our concerns. PAWS also reserves the unqualified right to remove the pet if the conditions are deemed, in PAWS's sole discretion, to be unsuitable for a PAWS animal.
If said pet cannot be kept to live out its life in the care of the person(s) signing below, PAWS will assist in finding a new home. The signee(s) is (are) precluded from transferring ownership of the pet until the PAWS office is notified and authorizes this change in ownership. The new owner must fill out a new adoption form to be kept on file with PAWS.
NO ANIMAL ADOPTED THROUGH PAWS SHALL EVER BE TAKEN TO ANIMAL SHELTER FOR ANY REASON.
